Patient Story: This patient is a teenage who had breathing difficulty, a tilted nose, and a dorsal hump. She underwent surgery to correct these issues and has been pleased with her results. Her case demonstrates the fact that rhinoplasty can be performed as early as age 12 in women and 13 in boys so long as they are sufficiently emotionally and physically mature. While, this is the minimum age many patients and parents wait until the mid-teen years to undergo these types of procedures.
Revision Rhinoplasty 07

4 1/2 months after revision rhinoplasty. She had 2 prior surgeries – this was her second revision. Her first 2 surgeries were not done by Dr. Weinfeld. These type of complex revisions are some of the most challenging surgeries in plastic surgery. Revision rhinoplasties almost always require rib graft which was true in this case. Sometimes ear cartilage is required to create additional details. This patient also had pre revision micro fat grafting to the skin of bridge of the nose (dorsum) in order to prepare it for the third surgery by restoring the natural padding which is protective. Revision goals: smooth the bridge, de-rotate the turned-up tip, lengthen the over shortened nose by rebuilding the lower septum to provide better size balance with the rest of the face. At 4 1/2 months after multiple surgeries we know there is a lot of swelling still present so the results will get even better with more time.

Patient Story:
This patient is in her mid Forties. She was referred by her husband who is a doctor in Austin, Texas and knew Plastic Surgeon Dr. Weinfeld by reputation. She stated outright that she was interested in a facelift but did not feel she was quite ready. Instead she wanted to focus on her upper face first as the next best thing. She wanted to address upper lid excess skin and lateral hooding. With regard to her eyebrows she emphasized that she did not want a “surprised” look. She was more interested in position stabilization and smoothing of the temples more so than significant elevation and shaping. Her goals for her lower lids were to reduce the fine wrinkles and the appearance of “bags.” Following her surgery she has been very happy with the natural and subtle brow lift in addition to the reduction of excess upper lid skin. Most of all she likes lower lid smoothing and the blending of the lid cheek junction afforded by the septal reset and fat transposition. The subtle elevation of the upper malar (cheek) tissue through the subciliary (open approach) yielded benefits in the midface as well, benefits rarely seen with alternative approaches such as transconjunctival lower lid blepharoplasty. It is for that reason that when performed appropriately, the lower lid subciliary lower lid blepharoplasty inherently contains a natural element of upper cheek lift. The subciliary approach to lowed blepharoplasty involves an incision just below the lower lashes which results in a scar that is difficult to perceive. It is a more complex approach to blepharoplasty with which not all Plastic / Cosmetic Surgeons have experience. However, due additional training and experience in Oculoplastic Surgery, Dr. Weinfeld is well versed in this and other advanced techniques for the eyelids, brows, temples, and upper cheeks.

This 60-year-old woman was photographed four months after undergoing a deep plane facelift and neck lift. Her primary goal was to achieve a natural, refreshed appearance, focusing on improving the lower face, jowls, and loose neck skin. She preferred not to alter the upper third of her face and therefore chose not to undergo a brow lift or blepharoplasty.
While periorbital procedures (such as eyelid or brow surgery) can enhance overall facial balance, they are not always necessary for beautiful results. This case demonstrates how a deep plane facelift and neck lift alone can restore youthful definition, refine the jawline, and smooth the neck—creating a naturally rejuvenated look that aligns perfectly with the patient’s goals.

Patient Story: This patient originally from South America came to visit Dr. Weinfeld in his Austin office to discuss rhinoplasty. Both she and her mother were very particular about the look that was desired. She wanted her nose to be more delicate and refined. She underwent surgery and was pleased with the results. She later referred her sister, from Washington, D.C., to Dr. Weinfeld to undergo Breast Augmentation.

20 week (short term) follow up for rhinoplasty revision, liposuction and renuvion of the lower face and neck, and burr reduction genioplasty. The nose is still swollen and will be for a while because this is her second surgery, but the improvement in shape is evident. Pay close attention to the more symmetric nostrils and tip shape. The lower face, jaw line, and chin results are impressive even at this early date. Liposuction reduced the fat and renuvion tightened the skin creating a “tighter fit” over the mandible giving wonderful jaw definition. The chin is rounder and more feminine. She looks like she lost 15 or more pounds but tells us she is the same weight.

Patient Story
This patient is a women in her late fifties who wanted to look more energetic, refreshed, and somewhat younger. She started the consultation with Dr. Weinfeld (Austin, Texas) by stating that she did not feel she was ready for a facelift yet and needed a procedure that allowed for a very quick recovery. She identified her upper eyelids as the area of greatest concern. She elected to undergo an upper lid blepharoplasty (referred to as a “upper lid lift” by the general public) with local anesthesia only. She reported the procedure itself and the recovery had far less discomfort than she expected. Given that the procedure was performed under local anesthesia she had minimal bruising and swelling. She returned to work almost immediately after the procedure.

She had difficulty breathing due to septal deviation and turbinate hypertrophy but also desired natural improvement in the shape of her nose. The scar can be a little more visible in men. And women it is located where the breast meets the chest in the lower portion of the breast in the inframammary fold, and therefore is often not seen at all. Cadaveric Rib Cartilage is also an option for many patients.
